A former CEO of the Weeneebayko Area Health Authority (WAHA) in northern Ontario has expressed unease over the organization’s loss of accreditation compared to when he led it 16 years ago. Pat Chilton shared his concerns following the tragic death of 11-month-old Jake Koostachin from Attawapiskat at the Fort Albany Hospital.
Chilton, along with a former chief of staff at the hospital, has highlighted persistent systemic issues like doctor shortages. While neither has direct knowledge of the circumstances surrounding Jake’s passing, they advocate for an investigation to provide clarity to the grieving family.
The WAHA oversees various healthcare facilities in northern Ontario, including the general hospital in Moose Factory, hospitals in Fort Albany and Attawapiskat, and nursing stations in Kashechewan and Peawanuck. Jake’s family continues to seek answers regarding the care provided to the young Cree child before his tragic demise.
Jake’s mother recounted taking him to the Fort Albany hospital with a fever, where they waited for hours for a doctor’s call from Moose Factory, located over a hundred kilometers away. Tragically, Jake deteriorated rapidly, and efforts to transfer him were made, but he passed away shortly after.
Accreditation Canada, responsible for assessing quality standards in healthcare organizations, has not listed WAHA among its accredited institutions since April 2026. This has raised concerns among former executives like Chilton, who emphasized the importance of accreditation in ensuring transparency and accountability in healthcare services.
Dr. Murray Trusler, WAHA’s former chief of staff, emphasized the challenges of diagnosing infant diseases and the broader issue of doctor shortages, particularly in Indigenous communities. The Mushkegowuk Council has called for an inquest to examine the circumstances surrounding Jake’s tragic death, highlighting the need for comprehensive systemic reviews.
Efforts to reach WAHA for comments on the matter, including staffing levels and accreditation status, have yet to receive a response. The community and stakeholders await further developments and accountability in the wake of this heartbreaking incident.
