As reports emerged about an Ebola outbreak in the Democratic Republic of Congo more than a week ago, Jason Kindrachuk expressed confidence in the local responders and health-care workers he has collaborated with in the past. However, his optimism about containing the outbreak has waned in recent days. The number of suspected cases has surpassed 1,000, with numerous fatalities concentrated mainly in northeastern Congo and seven cases reported in neighboring Uganda.
Experts believe the actual number of cases is likely much higher, as the outbreak may have been escalating for weeks before it came to the attention of global health authorities. Furthermore, the specific strain of Ebola involved, the Bundibugyo variant, lacks any authorized treatment or vaccine, adding complexity to the situation.
University of Manitoba associate professor Kindrachuk emphasized the urgency of identifying suspected cases swiftly to contain the outbreak effectively. He cautioned that the crisis is a race against time, noting that the situation may persist for longer than a few weeks.
In a virtual briefing with African officials on Monday, the World Health Organization Director-General, Dr. Tedros Adhanom Ghebreyesus, acknowledged the challenges posed by the delayed detection of the outbreak. He expressed concerns that the epidemic, now rapidly evolving, will likely worsen before showing signs of improvement.
The obstacles faced by health-care workers are formidable. In addition to the absence of a vaccine for preventing new infections, violent incidents in Congo’s troubled Ituri and North Kivu provinces, coupled with widespread distrust of external authorities, have hindered response efforts. Recent events, such as attacks on health facilities and Ebola patients fleeing treatment centers, have exacerbated the situation.
Efforts to combat the outbreak are complicated by logistical challenges. The vast territory of Congo, ongoing conflicts, and difficulties in tracking infections and deploying resources pose significant hurdles. Remote regions are particularly challenging to access due to the lack of infrastructure, making it harder to monitor and respond effectively to the outbreak.
Moreover, ongoing interruptions in care and contact tracing have impeded early identification and treatment of Ebola cases. The unstable environment in the epicenter of the outbreak, Ituri province, has forced health workers and humanitarian personnel to flee violence, hampering containment efforts. Dr. Rob Fowler, a critical care physician from Toronto’s Sunnybrook Hospital, highlighted the disruptions in contact tracing caused by instability and emphasized the need for suitable personal protective equipment, especially in Africa’s demanding climate.
Kindrachuk and other Canadian experts familiar with Ebola outbreaks stress the urgent need for effective vaccines tailored to non-Zaire forms of the virus. Development and testing of experimental vaccines are underway, but the process takes time. They lament the societal and financial barriers hindering the availability of approved vaccines for various strains of the virus.
